BEFORE YOU BEGIN: If this is an emergency that involves an imminent risk of harm to self or others, please contact local authorities or dial 9-1-1 prior to filling out a referral form.

These forms are designed to provide students, employees, faculty, vendors, visitors, or others with an online method to report various issues to the University. Referrals submitted via these online forms are immediately received through the system and reviewed the next business day by designated staff, who will determine the appropriate course of action.

Behavioral Misconduct

Behavioral Misconduct is a violation to the Student Code of Conduct that is not academic in nature, such as:

  • Disrespectful engagement
  • Email harassment
  • Aggressive confrontation
  • Any act that significantly disrupts educational/administrative process
  • Bullying or cyberbullying

  • Mental health concerns (anxiety, depression, etc.)
  • Food insecurities
  • Housing insecurities
  • Reported suicidal thoughts/plans
  • Made a threat to someone else or the broader community

Academic Misconduct

Academic Misconduct involves academic dishonesty in violation of the Student Code of Conduct, such as:

  • Plagiarism
  • Cheating/collusion
  • Recycled assignments
  • Use of prohibited sources
  • Purchased assignments
  • Use of surrogate(s)

The NU Academic Success Center team supports students through the process of developing their identities as scholars and lifelong learners. Through scaffolded academic coaching and tutoring, we assist students in gaining the confidence needed to achieve academic success and empower them to become active participants in their learning journey.
If you are experiencing the following situations with a student, the Academic Success Center team is here to help.

  • Low Academic performance/falling behind
  • Academic skill development needs
  • The student is requesting additional support
  • Connect a student to tutoring or coaching

The Office of Student Affairs (OSA) provides assistance when normal departmental processes have failed to resolve student problems or when there is not a well-defined procedure to address a unique concern. OSA analyzes the issue and develops a creative roadmap for possible solution(s), adhering to the Policies and Procedures published in the NU catalog.

COVID-19 Concern

This form is solely to report symptoms/diagnosis/exposure to COVID-19 for members of our community (student, staff, or faculty) who have been onsite at either an NU campus or clinical site in the last 30 days or plan to be in the next 30 days.

NOTE: This form is only open to NU staff and faculty. If you are a student with a concern, please email studentwellness@nu.edu.

Information Security

Students, please send an email to informationsecurity@nu.edu to let the Information Security Team of any issues regarding Information Security such as suspicious activity, passwords  or systems apparently compromised, impersonification or unauthorized use of account credentials.

For any malware, virus or university systems impacting functionality or availability, please send an email to Help Desk at help@nu.edu, they can assist you immediately.
